Social prescribing recognises that health is heavily determined by social factors such as poverty, isolation and loneliness. Social prescribing offers GPs and other health professionals a means of referring people to a range of non-clinical community supports which can have significant benefits for their overall health and wellbeing.
This service is available in Carlow County, where a social prescribing link worker engages with a patient to support them with a plan which can be a series of signposting or referrals to support in the community, in order to improve their health and wellbeing.
The Social Prescribing Service in Carlow would like to gather information to build a directory of local services, activities and events in order to better improve this very important service.
